Monroe County Community Health Needs Assessment

Monroe County Hospital invites you to review the following document "Monroe County Community Health Needs Assessment 2013". The purpose of the Community Health Needs Assessment (CHNA) is to describe the health of the community by presenting information on health status, community health needs, resources, and analysis of local health problems. The CHNA is the foundation for improvement planning in the local health system.

Monroe County Hospital was an active participant in this CHNA in collaboration with the Monroe County Health Department, Department of Family and Children Services, Monroe County Board of Education, and a number of other local businesses and organizations in Monroe County. This collaborative approach enabled the group to identify areas where the various organizations might work together to achieve needed improvements. Monroe County Hospital will utilize this assessment as a guide over the next three years for providing programs to address identified community health needs in support of the collaborators and with the mission of our organization" to provide compassionate community healthcare that meets the public's expectations."

We do not have adequate resources to solve all problems identified. Some issues identified are beyond the mission of the hospital and action is best suited for a response by others. Some improvements will require personal actions by individuals rather than the response of an organization. We view this plan as providing guidance around how we, along with other area organizations and agencies, can collaborate to make the best medical care available to the citizens of our community.

Monroe County Hospital will conduct or collaboratively engage in a community health needs assessment at least every three years. The resulting report is developed in response to a federal requirement to which we must acknowledge and respond.
